What we do

  • Define required functions, then identify functional failures, failure modes and causes, local and system-level effects, existing controls and detection methods, consequences, current maintenance response and recommended risk reduction.
  • Where appropriate, add criticality or risk ranking to produce an FMECA.

Why clients need it

  • Many maintenance programmes are built around equipment lists or OEM recommendations rather than the failures the organisation is trying to prevent — leaving important failure modes untreated while resource is spent on tasks that control no meaningful risk.
  • FMEA also exposes hidden failures, weak controls, operating vulnerabilities and design issues that do not appear in maintenance records.
